손목 터널 증후군이란, 손목 터널이 좁아져 정중신경을 눌러, 신경이 지배하는 손, 손가락의 영역의 이상감각과 근력저하, 근육 위축이 발생하는 현상입니다.

손목 관절의 작은 뼈들과 손목을 가로지르는 단단한 인대 (횡 수근 인대) 사이의 공간이 손목 터널입니다. 이 속으로 손가락 운동과 연관된 힘줄, 신경이 지나갑니다. 이 때 어떤 원인에 의해 손목 터널 안 공간이 좁아져 손가락과 손바닥을 지배하는 정중신경이 눌리게 되면 여러 가지 감각, 운동과 관련된 증상이 나타납니다.

제일 흔하고 뚜렷한 증상은 손바닥과 손가락의 저림, 찌릿함, 통증 등 감각이상 및 근력 약화 등입니다. 신경이 눌린 정도가 심하지 않을 경우 증상 역시 일상 관리와 함께 적절한 치료로 잘 완화될 수 있습니다. 하지만 초반에 잘 관리하지 못하거나 정중신경이 눌린 정도가 심하다면 엄지 손가락 두덩 근육이 위축되거나 손의 쥐는 힘이 떨어지는 등의 증상이 나타날 수도 있습니다.

손목 터널 증후군의 원인은 아직 명확하게 밝혀지지 않았지만, 손과 손목의 과도한 반복 사용, 손목 골절 후 변형, 염증성 관절질환, 당뇨/갑상선기능저하증 등의 대사질환, 비만, 임신 등이 연관 요인으로 알려져 있습니다.

손목 터널 증후군은 병력 청취 및 신체 검진, 근전도/신경전도 검사 등의 전기생리학적 검사를 통해 진단됩니다. 신경 압박 정도가 심하지 않을 경우, 증상 역시 일상 관리와 병행한 적절한 치료로 좋은 성과를 얻을 수 있으나, 엄지 손가락 두덩 근육이 위축되거나 손의 쥐는 힘이 떨어지는 등 근력 저하/근육 위축 증상이 있을 때에는 후유증을 방지하기 위해 수술적 치료를 우선으로 고려하게 됩니다.

한의학에서는 손목 터널 증후군에 대해 침 치료를 위주로 합니다. 손목 주변 및 연관된 경혈에 침을 놓아 통증을 개선시키고 기능적 회복을 촉진하는 효과가 제기되었습니다. 최근의 뇌영상 연구에서는 침 치료를 받은 손목 터널 증후군 환자의 뇌 신호가 건강인과 비슷하게 변화했음을 보여주었습니다.

장시간 과도한 손목의 반복작업, 중량물 작업으로 인한 손목 손상을 피해야 하며, 가벼운 증상이 있을 때 바로 의료기관을 방문하여 적절한 치료를 제 때에 받는 것이 건강 유지에 필요합니다.
